YOUR FIRST DENTAL VISIT
Your schedule is important to us and keeping the office running on time is always a top priority too. Please arrive 15 minutes before your scheduled appointment. This will allow us to finish any paperwork and stay on schedule with your appointment.
Please bring the following to our Yulee, FL location when you arrive for the first time:

Your insurance cards and/or claim forms

Your finished new patient forms (if you’ve chosen to complete them ahead of time)

A list of any current prescriptions

Any relevant medical/dental history
